Apply for First Registration
Apply for the first time Occupational Therapist with the SLMC. Should holds a certificate of proficiency as an Occupational Therapist issued by the College Council.
Replacement of damaged /misplaced ID card
If an applicant has misplaced or damaged the original SLMC ID card, they may apply for a replacement ID card.
Changing of Information recorded in the SLMC Database.
Applicants who wish to update the information maintained by the Sri Lanka Medical Council (SLMC), including their name, address, or National Identity Card (NIC) number, may apply for a Change of Information.
Apply for Certificate of Extract of Registration in place of damaged/ misplaced of Registration Certificate
Applicants who have misplaced or damaged their original SLMC certificate may apply for a Certificate of Extract of registration.
